You forgot

 [foobar] Re: [foobar] (was [foobar] Re: [foobar] (was [foobar] Re: [foobar]))

Which I've actually seen, with some broken list managers.  Mailman
automatically avoids duplicating the [foobar] string.  But that
doesn't work when you've changed strings.
