[LMH]doc/tape update

There are several new scanned documents at www.spies.com/aek/pdf/ti/explorer
and I was able to read a few 87'ish tapes for R3.0 and R3.2

Unfortunately, the boot tape has a bad block, so I'm still trying to locate
6.0 or 6.1 boot tapes so that people with scrogged discs can get their lispms
running again.

Even if I get a read error on another 6.0 distribution tape, it probably won't
be on the same block, so I can reconstruct the image from a combination of the
two 6.0 tapes.
